This Service Agreement (“Agreement”) is entered into on [Date], by and between:

Consultant: Theodora M. Micklos, BSN, RN d/b/a Precision Medical Review (“Consultant”), and

Client: [Attorney/Firm Name] (“Client”).

1. Scope of Services

Consultant will provide legal nurse consulting services, including but not limited to:

• Case merit review
• Chronology and case analysis
• Standards of care research
• Expert witness referral
• Written reports and summaries

All services will be performed according to Consultant’s professional nursing expertise and delivered in a timely, attorney-ready format.

2. Fees & Payment

• Retainer: A retainer of $500 is required before services commence. Retainer will be applied to the first invoice.
• Flat Fees:
  • Case Merit Screening: $500 flat fee
  • Chronology (up to 12 hrs): $1,800 flat fee
• Hourly Fees: $150/hour, billed in 30-minute increments, applied after flat-fee caps are reached or for hourly services.
• Expert Witness Referral: $200 flat fee.
• Rush Requests: Work requiring completion within 72 hours is subject to a 25% surcharge.
• Payment Terms: Invoices are due Net 15. Late payments are subject to a finance charge of 1.5% per month.
